Transition Professional Referral

We welcome referrals from professionals, for young adults who are likely to have care and support needs following their 18th birthday. Referrals can be made for young people from the age of 14 years.
The young person must have an active Education, Health Care Plan (EHCP), and a diagnosed learning, physical or sensory disability and/or autism for our team to be able to accept the referral. If the young person does not have an EHCP and one of the listed diagnoses, then you should refer to either the Learning Disabilities and Autism Team, Adult Social Care Locality or Mental Health Social Care.
Prior to making a referral to our team, please ensure all the questions in the form are completed. If all information requested is not provided, we may not be able to accept your referral.

You will need to create an account and log into our portal to submit the referral. Once you are logged in, you'll have the ability to:

• Save the form and retrieve it at a later point
• Easily access any forms you've submitted in the past.
• Obtain hard copies or digital versions of your submitted forms to print or save into your files
• See any messages from us requesting further information or to confirm that we have received and processed the referral.

Please note, you will need the following information and documentation to upload before you can complete and submit your referral.

For each young person we require:

• Copy of the latest EHCP

• Formal evidence of diagnosis

• Details of education provision, future plans and achievements to date.

• Family contact details

• Contact details of all involved professionals.
