Portal Carers Wellbeing Break payment referral

Caring for someone can be hard work so taking time for yourself is important. The carer wellbeing break payment could help you do this. It is a one-off £300 payment to help you take a well-deserved break from your caring role whether that's to enjoy an activity, hobby or anything else that helps to give you a break away from your caring role. The carer wellbeing break payment is open to all adult carers of adults in Surrey who :

• are 18 years or older
• look after someone who lives in Surrey
• have not had a previous carer's payment, for example from a GP
• have not used a carer break offer
• are not getting support from us

The person the carer looks after must:

• be 18 years or older
• live in Surrey
• not have a social worker

This form is for carers of adults. If you are caring for a child you can find information on our parent carer pages. You can also apply for a payment from Parent Carers Payments from Action for Carers or Surrey Short Breaks for Disabled Children

You can apply as:

• the carer
• a friend or family member, with the carer's permission
• a professional

We want to make sure this is the right offer. We will need the carer's:

• contact details
• proof of address
• Carers will be asked to submit ID documents. Without these your application will not be eligible
• how taking a break will help

What forms of ID you need to provide: One item must be from list A and 1 item from list B.   List A - proof of identity

• valid, current passport
• valid, current photocard driving licence
• birth or adoption certificate
• current biometric residence permit
• Department of Work and Pensions (DWP) letters

List B - proof of address Council Tax bill dated in the last 12 months

• bank or building society statement dated in the last 3 months
• utility bill – gas, electricity, water or phone bill dated in the last 3 months
• benefits statement dated in the last 3 months
• DWP letters

Send your ID documents once you have submitted your application to asc.infoandadvice@surreycc.gov.uk The more you tell us, the better. We may need to contact you to understand what support you need and if there are other ways to assist you with the full range of local community and carer support services in Surrey. The form should take around 10 minutes to complete. You will need to register with an email address. Our privacy notice explains how we store and use your information.

Please Note. It may take up to three working days to process this referral. Working days are Monday to Friday, 9am to 5pm. If the referral is urgent and requires an immediate response, please submit the referral form, make a note of the reference number, and call our Information and Advice Line on 0300 200 1005 to alert our staff to the urgency.
