If you are known to adult social care and in receipt of services we will hold all information relevant to your care and support on our electronic database.
Registering for an online account will allow us to send you information directly from our database, and you will be able to send us information which will go straight onto your social care record.
Information that might be shared with you via your social care account could include:
- Assessments, Support Plans and Reviews which we have agreed with you
- New forms for you to complete and return to us
- Messages to or from your key worker
- Details of any changes to your information such as a new address
All information held about you will be stored in accordance with the Data Protection Act 1998. Further information about how we
